If You Have a Concern
Notice of Dispute and Grievance Procedures
You have the right to raise a concern about your care, your treatment, our staff, your access to services, our facility, or your rights, at any time. Telling us will not affect your services.
How to Tell Us
You may raise a concern in whichever way is easiest for you:
- Speak to any staff member, in person or by telephone.
- Write it down. You may use our Dispute Submission Form, or any format you choose.
- Send it by email, or submit it through our website.
- Ask a family member, guardian, or representative to raise it for you.
- Submit it anonymously.
You do not need to use a special form or particular wording. If you tell us verbally, we will write it down for you and offer you a copy to review and correct.
What Happens Next
We will confirm we received your concern, look into it, and give you a response no later than thirty (30) business days from the day you first tell any staff member about it. Our response will explain what we understood your concern to be, what we did to review it, what we concluded, and who to contact with questions.
Who Can Help You
The staff member below is designated to help you raise and resolve a concern: putting it into words, submitting it for you, explaining your rights, and telling you how to file with an outside agency. Help is free, including interpretation.

Responsibilities: Available to assist you in resolving disputes; documents verbal concerns in writing; coordinates the review; makes sure you receive a response within thirty (30) business days; and tells you how to file with the State.
If your concern is about the Designated Representative, you may bring it directly to the Executive Director. You are never required to raise a concern with the person it involves.
You Can Also File With the State
You may file a grievance with the Colorado Behavioral Health Administration (BHA) at any time. You do not have to tell us first, and you do not have to wait for our response.

We will not retaliate against you.
Your services will not be reduced, delayed, suspended, or ended because you raised a concern. Retaliation against anyone who raises a concern, or who takes part in a review, is prohibited.
This notice is available in the languages commonly used by the communities we serve. Ask any staff member for a copy in your language, in large print, or in another format.
